Registered Nurse - Case Manager

Role Overview This nursing position involves assessing, developing, and evaluating patients' discharge planning needs in collaboration with an interdisciplinary team. Key Responsibilities Ensuring compliance with discharge planning standards, identifying barriers to length of stay, assessing high-risk patients for readmission, and collaborating on care transitions. Additional duties include utilization management, ED case management, and acting as a liaison with post-acute providers. Qualifications & Skills Requires 3-5 years of acute care or ICU nursing experience, strong communication skills, attention to detail, and self-motivation. A graduate of an accredited nursing program and current RN licensure in applicable states are mandatory. Physical & Certification Requirements Includes physical capacity for lifting and movement, with necessary RN licenses for respective states. Work Environment Fully onsite, day shift, Monday to Friday, with no on-call duties.
