Pride Health is hiring a RN - Home Health to support our client's healthcare facility in Saint Petersburg, FL.

Job Details:
• Schedule: Monday-Friday, 8:00 AM - 4:30 PM, with on-call/weekend rotation
• Mileage Reimbursement: Yes, between patient visits
• Location: Saint Petersburg, FL
• Coverage Area: Pinellas County
• Job Type: Contract
• Guaranteed Hours: 40 hours/week
• Pay Range: $52-$54/hour
Pay offered is based on experience, expertise, credentialing, and education.

Duties:
• Provide skilled nursing care to patients in their homes, including comprehensive assessments, OASIS assessments, medication reconciliation, and individualized care planning.
• Coordinate patient care with physicians, patients, families, and interdisciplinary team members while maintaining accurate and timely documentation using the HCHB EMR.

Requirements:
• Active RN License in the state of practice
• Current BLS/CPR Certification
• Ability to complete OASIS assessments and manage patient care independently

Preferred Qualifications:
• Home Health experience preferred
• Experience with HCHB preferred
