The Judge Group is Hiring: Per Diem Nurse Practitioner / Physician Assistant Location: Herkimer County, New York Update: Preferred Experience: Primary Care Provider (PCP) or in‑home health experience; ideally both. Minimum Monthly Requirement: 15 completed visits. Providers must be credentialed with Excellus, NYS Medicare, and Medicaid to avoid credentialing delays. Travel Requirement: Up to a 50‑minute radius. Position Summary The Provider performs in‑person, no‑cost annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act members. Assessments take place within the member’s personal living environment and must meet all health plan expectations and quality standards. Schedule & Compensation Hours: Flexible daytime and evening availability; appointments scheduled for 60–75 minutes. Compensation: Market‑rate payment per completed assessment. Mileage: Reimbursed after the first 30 miles driven per day at the current IRS rate. Essential Functions Successfully complete all required self‑paced and instructor‑led training within designated timelines. Provide 15+ appointment slots per month. Deliver high‑quality assessments aligned with member satisfaction and health plan standards. Submit chart notes within 24 hours of each visit. Address charting queries within 48 hours. Maintain strong communication with clinical leadership. Attend periodic training sessions and position‑related meetings. Be comfortable entering members’ personal living spaces. Physical Requirements: Ability to lift 20 lbs. Ability to bend or kneel for extended periods during assessments. Competencies Strong computer proficiency and experience with electronic medical records. Understanding of CMS and ACA risk‑adjustment models. Ability to document findings accurately and thoroughly. Effective verbal and written communication with members and clinical teams. Strict adherence to HIPAA and confidentiality standards. Ability to make critical decisions within policy guidelines. Adaptability to changing conditions and responsibilities. Positive, collaborative approach with members and clinical staff. Ability to proactively anticipate, address, and resolve member, provider, and health plan issues. Education & Experience Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP) OR Master’s degree from an accredited PA program. Home health experience required within the last 5 years. 1 year of in‑home assessment experience highly preferred. Certification & Licensure Current RN and NP or PA licensure in the state of practice. Board certification through ANCC, AANP, or NCCPA. Current Medicaid ID for the state. Current Medicare PTAN. Current BLS certification. Valid, unencumbered state driver’s license. Proof of sufficient auto insurance coverage for the state in which appointments occur. #J-18808-Ljbffr The Judge Group
