Home-Based Hospice RN & Case Manager
Gentiva
Gentiva Hospice is seeking a dedicated RN Case Manager to provide in-home hospice and palliative care. You will lead patient assessments, deliver hands-on nursing care, and coordinate with physicians, social workers, and chaplains to ensure compassionate, personalized plans.
Ideal candidates are a Registered Nurse with 1+ year hospice/home health experience, CPR certified, and a passion for supporting patients and families during end-of-life care.
