This role offers flexible daytime scheduling and territory assignments aligned with your home location—helping you maintain work-life balance while minimizing travel time. Position Overview The Registered Nurse (RN) plays a critical role in coordinating and overseeing care for members receiving services through the ADvantage Program. This position focuses on comprehensive assessment, care planning, and ongoing case management to ensure members receive safe, effective, and individualized care in their homes. You will collaborate closely with members, families, and interdisciplinary teams to ensure continuity of care and optimal outcomes. What We Offer Mileage reimbursement ($0.46 per mile) Flexible daytime schedule — no nights or 12-hour hospital shifts Territory assignments designed to reduce drive time Supportive and accessible leadership team Growth opportunities within an expanding organization Comprehensive benefits package (Medical, Dental, Vision) 401(k) retirement plan Life and supplemental insurance options Key Responsibilities Conduct comprehensive in-home assessments, including functional and cognitive evaluations Develop, implement, and monitor individualized care/service plans Coordinate services with healthcare providers, community resources, and support systems Provide direct nursing care as needed Educate members and families on disease management, safety, and available resources Monitor changes in condition and initiate appropriate interventions Ensure timely, accurate, and compliant documentation in accordance with agency and program requirements Participate in care coordination meetings and interdisciplinary collaboration Support care transitions and discharge planning when appropriate Maintain compliance with Oklahoma ADvantage and Medicaid (MSU) guidelines Qualifications Active and unrestricted Oklahoma Registered Nurse (RN) license Current BLS certification Valid driver’s license and reliable transportation Ability to work independently and manage time effectively Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Knowledge of community resources and care coordination practices preferred Familiarity with ADvantage / MSU guidelines preferred, but not required What Makes You a Great Fit You value autonomy but also appreciate a supportive team You are organized, detail-oriented, and clinically confident You are passionate about keeping patients safe and independent at home You thrive in a role that blends clinical care with coordination and advocacy #J-18808-Ljbffr Med-Corp Health Services
