Key Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
Provide direct occupational therapy services to patients to restore, maintain, or improve functional abilities
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory requirements
Collaborate with interdisciplinary care team members to ensure coordinated patient care
Educate patients and family members on therapeutic techniques and home exercise programs

Required Qualifications

Graduate of an accredited Occupational Therapy program
Current and valid state licensure or eligibility to practice as an Occupational Therapist in Minnesota
Current NBCOT certification
Experience working in a skilled nursing facility (SNF) setting preferred

Preferred Experience / Skills

Prior experience in a SNF or long-term care environment
Familiarity with MDS documentation and Medicare/Medicaid guidelines
