Lead Birthing Families Navigator — Two-Site Leader

Erie Family Health Center

Erie Family Health Center is seeking a Team Leader Birthing Families Navigator to oversee direct client services and supervise a team of Birthing Family Navigators across two sites: Erie Helping Hands and Erie HealthReach Waukegan. The role partners with medical teams, conducts assessments, develops care plans, makes referrals, and ensures documentation in EHR and funder data systems, with outreach and home visitation as needed. #J-18808-Ljbffr Erie Family Health Center
