Job Title: Physical Therapist
Location: Greater Jacksonville, FL
Type: Permanent / Direct Hire
Responsibilities:
- Provide physical therapy to patients in their homes.
- Perform initial evaluations, re-evaluations, ROC, recertifications, and discharge assessments.
- Develop and update patient care plans.
- Monitor patient progress and adjust treatment as needed.
- Work with PTAs who handle many routine visits.
Requirements:
- Valid Physical Therapist (PT) license .
- Strong knowledge of anatomy, physiology, and medical terminology.
- Strong patient-care and communication skills.
- Home health experience preferred but not required .
- Ability to provide individualized, patient-centered care.
