Hospice Social Worker - Palliative Care & Family Support
MJHS
MJHS seeks a licensed MSW to provide psychosocial support to Hospice patients and families across inpatient and home care settings, with emphasis on bereavement care. You will collaborate with an interdisciplinary team and help plan patient-centered goals. Requirements include NYS MSW licensure, one year in healthcare social work, and proficiency with Word and Excel.
