This role involves assessing client needs, developing care plans, monitoring progress, and collaborating with interdisciplinary teams to achieve optimal outcomes. Key Responsibilities include: Conducting comprehensive assessments of client needs, including medical, social, and financial factors. Developing, implementing, and updating individualized care or service plans. Coordinating services across providers, agencies, and community resources. Monitoring client progress and adjusting plans as needed. Advocating for clients to ensure access to appropriate services and resources. Maintaining accurate and timely documentation in compliance with organizational and regulatory standards. Educating clients and families on available resources and care plans. Facilitating communication between clients, families, and healthcare or service providers. Ensuring compliance with policies, procedures, and applicable regulations. Participating in interdisciplinary team meetings and case reviews.
