Currently we are looking to fill the vital role of Population Health Nurse.

JOB SUMMARY:

• Coordinates research data, community physicians, hospital resources into a seamless model of access and care benefiting patients, participating physicians, and family members.
• Assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes.
• Uses critical thinking skills to ensure efficiency and assist with troubleshooting.
• Is an integral part of the patient care team. This is a full-time, weekday, day shift position, requiring the ability to work independently.
• Must be able to adhere to the attendance policy and have good attendance history.
• The ability to multi-task and prioritize to handle stressful situations is required for this position.

QUALIFICATIONS:

• Requires current Utah State LPN or RN License.
• 3 years prior experience in patient education, nursing, planning, case management, or similar experience preferred.
