Registered Nurse Case Manager

Registered Nurse Case Manager - RN | Legacy Lifecare VNA/Hospice $41.00 - $43.00/hour, based on experience Full-Time | 40hrs per week Monday – Friday | 8:00AM – 5:00PM | Occasional On-Call Rotations Legacy Lifecare VNA and Hospice not only provides compassionate and comprehensive care, but we are a leader in the industry when it comes to the value we place on resident care and employee appreciation! Delivering Solutions that Matter: Provide kind, compassionate care to residents in home or assisted living settings. Observe residents and chart/report changes such as response to medication or treatment. Perform basic resident care or treatments, including temperature and blood pressure monitoring. Support palliative and hospice care needs, including pain management, symptom control, and emotional/spiritual support. Qualifications for Success: Current, unencumbered MA RN License. Minimum 1-year RN Experience Prior Experience in Home-care, Hospice, rehabilitation, or long-term care setting preferred. Reliable transportation and the ability to travel to patient homes or hospice facilities. Ability to communicate fluently in English - both verbally and written.
