Weekend Home Health RN

Seeking a compassionate and experienced RN to join their extraordinary care team. This full-time position involves providing management and delivery of patient care plans.
The role requires on-call availability on weekends and local travel to patient locations. The successful candidate will have an associate degree, an RN license in the state of work, and at least one year of RN experience.
Key Responsibilities
• Lead and collaborate to provide management and delivery of patient care plans
• Spend ~20% of the day driving to/from patient locations
• Consistently meet critical deadlines for charting
• Self-manage time and schedule Schedule and Shift Details
Schedule: Friday, Saturday, Sunday on-call 5p-1p, Scheduled to see patients during the day Saturday/Sunday (8 units per day)
