Responsibilities

• Perform patient assessments and develop individualized care plans.
• Coordinate discharge planning and transitions of care.
• Collaborate with physicians, social workers, and the multidisciplinary team.
• Conduct utilization review and care progression activities.
• Identify discharge barriers and coordinate post-acute services.
• Document patient care and case management activities in Epic.

Requirements

Requirements

• Active North Carolina RN license.
• Minimum 2 years of RN experience .
• Recent acute care hospital Case/Care Management or Discharge Planning experience required.
• Strong assessment, care coordination, and critical thinking skills.
• Experience with Epic preferred.
