Required: NYS RN license, BLS AHA, discharge experience, 2+ years experience
Preferred: PRI, SCREEN, CCM/CMC, Milliman/InterQual, and prior case management experience
Skills: Initial assessments, discharge planning, UR activity, care coordination, readmission prevention, and payer reviews
