Job Description:

• Provide skilled nursing care to patients in their homes.

• Conduct assessments, develop and implement care plans.

• Monitor patient progress and document in EMR.

• Educate patients and families on disease management and medications.

• Coordinate with physicians, therapists, and other healthcare professionals.

Requirements

Requirements:

• Active RN license in [State].

• Minimum [1–2] years of nursing experience (home health preferred).

• BLS/ACLS certification required; other certifications a plus.

• Strong clinical, communication, and organizational skills.

• Ability to travel to patient homes and work independently.
