Key Responsibilities\n- \n
- Conduct comprehensive assessments of clients to identify their needs and develop tailored treatment plans. \n
- Create and maintain open communication channels with supervisors, staff, families, and other stakeholders to ensure seamless continuity of care. \n
- Provide a weekly schedule detailing client appointments, home visits, and needed observations for effective monitoring of patient progress. \n
- Coordinate essential home visits that involve chart reviews, staff interviews, and data analysis, ensuring a holistic approach to client care. \n
- Implement and evaluate the program plans effectively, monitoring treatment fidelity and outcomes. \n
- Actively participate in Interdisciplinary Team Meetings to share insights, collaborate on patient care strategies, and review ongoing cases. \n
- Document and complete monthly and quarterly progress notes to track the advancement of patient treatment goals. \n
- Organize and lead training sessions for staff to ensure competency in implementing program plans and maintaining high standards of documentation. \n
- Follow up with clients through interviews, home visits, and observations to support their ongoing needs and adjust treatment plans as necessary. \n
- Maintain organized records, ensuring that pertinent information is easily accessible to authorized parties. \n
- Serve as a liaison within the public and mental health communities to support service development and outreach initiatives. \n
- Assist in drafting and updating training materials to enhance program efficacy for individual clients. \n
This role involves working with a wide-ranging client population, including both adults and youth, addressing an array of mental health challenges. Your expertise will help individuals and families navigate complex emotional landscapes, including trauma, relationship issues, and behavioral disturbances. Qualifications\n- \n
- A Master's or Doctoral degree in Counseling, Psychology, or a relevant field is required. \n
- Active and unrestricted licensure as a Licensed Clinical Professional Counselor in the state of Illinois. \n
- Minimum of 2 years of relevant counseling experience, particularly within a clinical setting. \n
- A strong background or specialty in family therapy, couples therapy, trauma, or substance misuse is highly preferred. \n
- Thorough understanding of legal, ethical, and clinical standards of practice. \n
- Exceptional communication and interpersonal skills, with the ability to connect effectively with diverse populations. \n
- Documented ability to collaborate within a multi-disciplinary healthcare team. \n
- Technological proficiency, including experience with electronic medical record (EMR) systems. \n
