Palliative Care NP — Collaborative, Patient-Centered
Elevation Hospice of Colorado
Elevation Hospice of Colorado seeks a Nurse Practitioner to collaborate with physicians and teams to provide palliative care to patients facing chronic illnesses. Responsibilities include coordinating care, educating patients and families, and actively participating in the interdisciplinary group. The ideal candidate should possess a graduate nursing degree, hold an RN and APN license, and have excellent communication skills.
