Description

• Conduct evaluations for patients in both inpatient and outpatient settings while crafting personalized treatment plans
• Deliver physical therapy interventions with a focus on mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation team members to optimize patient outcomes
• Educate patients and their families on therapy goals, discharge plans, and exercises for home
• Maintain thorough documentation of evaluations, treatments, and patient progress in accordance with facility policies and regulations
• Engage in interdisciplinary rounds and care conferences to provide coordinated and comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Any additional specialized certifications (neurological, geriatric) are advantageous but not mandatory

Skills

• Experience in a clinical setting is highly preferred
