Job Description Job Description Home Health Physical Therapist Location: Boulder and surrounding areas Pay Rates: $75-$150 per visit, (depending on visit type), PLUSmileage and travel time compensation! Status: Part-Time We are looking for someone energetic, self-sufficient, hardworking, and reliable to provide care for clients in their home. Someone who is has the ability to multi-task, work under pressure with changing priorities, and effectively handle a heavy workload. Must be familiar or willing to learn Oasis documentation. Responsibilities Complete initial and ongoing comprehensive assessments as assigned. Provide interventions and therapeutic treatment Effectively communicate with other members of the interdisciplinary healthcare team to promote the coordination of patient care and planning for discharge. Participate in Quality Assessment and Performance Improvement (QAPI) program. Responsible for reporting on the condition of patient to the Rehab Director, and other appropriate staff members. Required Qualifications Current Physical Therapist (PT) licensure in the state of Colorado Current Basic Life Support (BLS) CPR or willing to obtain upon hire(demonstration course required) One year of PT experience Home health care experiencepreferred, but not required Pass a background check and drug test (we do to test for THC) No Covid Vaccination Required Company Overview Interim HealthCare is America's leading provider of home care, hospice and healthcare staffing.
