Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to home health patients in a 1:1 care model, visiting up to 6 patients per day
• Complete OASIS assessments and documentation accurately and in a timely manner
• Chart using EPIC Remote Client for Home Health and the Rover mobile application
• Manage a daily patient caseload across a defined geographic service area, with occasional drive times up to 60–90 minutes per day
• Maintain 30 productivity points per standard shift in accordance with program expectations

Required Qualifications

• Active Physical Therapist licensure in both Oregon (OR) and Washington (WA) required prior to start
• Minimum 1 year of skilled home health experience
• OASIS experience required
• Current Basic Cardiac Life Support (BCLS) certification
• Must have reliable personal transportation

Preferred Experience / Skills

• Experience with EPIC Remote Client for Home Health documentation
• Familiarity with the Rover mobile charting application
• Ability to efficiently manage a geographically distributed patient caseload
