Role Overview

In your role as a Home Health LPN/LVN, you will provide direct patient care under the supervision of a Registered Nurse, assisting in the implementation of care plans to ensure patients receive safe and effective treatment in their homes.

You will be a vital member of the care team, responsible for delivering treatments, monitoring patient conditions, and reinforcing education to promote successful outcomes.

Key Responsibilities

• Deliver direct patient care in line with the state Nurse Practice Act.
• Assist in implementing the care plan established by the RN.
• Provide skilled nursing services, including treatments, medication management, and wound care.
• Monitor patient conditions and communicate any changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Ensure infection control and proper use of equipment and supplies.
• Accurately document patient care in the EMR in a timely manner.
• Effectively communicate with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences, team meetings, and ongoing training.
• Support coordinated care through consistent communication with the care team.

Qualifications and Requirements

• Active LPN/LVN license in the state of practice.
• Graduate from an accredited practical/vocational nursing program.
• Current CPR certification.
• Preferred minimum of one (1) year of nursing experience (Home Health, Med-Surg, or Acute Care is a plus).
• Strong clinical and observational skills.
• Effective communication and patient education skills.
• Ability to follow care plans while recognizing and reporting changes in patient conditions.
• Basic proficiency with EMR systems.
• Valid driver's license and reliable transportation.
