This position is located in the Nursing Administration Department of the Veterans Home of California- Fresno and is responsible to coordinate the integration of interdisciplinary functions, transition planning, resident advocacy and level of care assessment for the Veterans who reside at the home.

• The position(s) require(s) a(n) Valid CA RN License. You will be required to provide a copy of your license prior to being hired.

STATEMENT OF QUALIFICATIONS

The SOQ serves as documentation of your ability to present information clearly and concisely in writing. In your SOQ, describe your experience, knowledge, and competencies as they relate to each of the items listed below. The items must be addressed and numbered in the same order as listed; do not consolidate responses. The SOQ must be no more than 2 pages , typed, using 12- point Arial font on standard letter-sized paper with one (1)-inch margins.

***PLEASE SUBMIT A STATEMENT OF QUALIFICATIONS DETAILING HOW YOUR EXPERIENCE MEETS THE FOLLOWING:

1. Experience in medical record review of patients admitted from acute hospital care to determine appropriate level of care using criteria approved by reviewing/licensing agencies (HCFA/CMA).

2. Knowledge of patient care, medical treatments and hospital procedures.

3. Knowledge of Medicare MDS scheduling for 5 day and 14 day MDA, Start/End of Therapy MDS, COT MDS and TYG's 1V utilization for SNF level of care reimbursements.

4. Ability to track/trend to ensure compliance for DOJ focused surveys to prevent Medicare/Medical fraud and survey citations which would affect the facitlity Star Quality Rating.

5. Knowledge of Medicare/Medical and private health care coverage benefits to help residents make informed decisions about healthcare.

6. Americal Board of Quality Assurance and Utilization Review certification.
