Job Summary
Physical Therapist (Outpatient) Pickerington, OH. The outpatient physical therapist evaluates assigned patients and develops, implements, and modifies individualized treatment plans aligned with patient needs, goals, and physician direction. This role includes clinical documentation, program design to support functional improvement, and coordination of discipline-specific therapy services while maintaining compliance with applicable standards. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days shift; 8 hours per shift; minimum 40.0 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ Year of outpatient clinical experience; CPR certification; active/applicable license for the state where work will be performed.
• Preferred Qualifications: Successful completion of an assignment with the organization previously; successful completion of an assignment with the organization previously (nice to have).
• Responsibilities:
  • Complete comprehensive evaluations of assigned patients and develop treatment plans with goals focused on progressing patients toward discharge within an established length of stay.
  • Provide medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in compliance with state, federal, and intermediary guidelines and clinic policies.
  • Ensure care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care.
  • Participate in the student program as assigned, providing a positive learning environment through appropriate supervision, development of learning objectives, and completion of student evaluations.
  • Create an individualized professional development plan, pursue professional growth through continuing education and professional activities, and apply learned information toward organizational objectives.
  • Adhere to the discipline-specific State Practice Act and other regulatory agencies; maintain professional licensure in good standing and complete state-mandated continuing education requirements.
  • Participate in the organizations performance improvement methodology by following action plans and providing feedback through staff meetings, focus groups, or with direct leadership as applicable.
  • Perform professional duties associated with patient care and the development and delivery of discipline-specific therapy services.
