The Hospice Admissions RN will meet with patients and families, explaining the hospice benefit, and facilitating their transfer onto hospice services. Hospice experience preferred but we are willing to train. This role is a calling for someone dedicated to patient-centered care at the end-of-life journey. Teaches, supervises and counsels the patient and family regarding care issues. Maintains accurate, complete, and timely documentation on patient's record. Encourages timely consideration by hospital staff and physicians for hospice as an alternative mode of care, assisting in identification of appropriate patients for the purpose of supportive care for terminally ill patients. Develops and maintains relationships with area Physicians, Nursing Homes, Hospitals, etc. and their staffs. Familiarity with community health organizations, practices and resources. Familiarity with the Hospice philosophy of care for the dying and in agreement with the principles of HOL care and a commitment to it.
