Responsibilities:
- Provide compassionate and individualized care to clients in their homes, focusing on their physical, emotional, and social needs
- Assist clients with daily living activities such as bathing, dressing, and grooming
- Monitor and report any changes in clients' condition to our care team
- Collaborate with our team of healthcare professionals to develop and implement personalized care plans
- Maintain accurate and up-to-date records of client care and services
- Act as an advocate for clients and their families, ensuring their needs are met and concerns are addressed
- Maintain a safe and healthy environment for clients and staff
Requirements:
- Current Certified Nursing Assistant (CNA) certification
- High school diploma or equivalent
- Ability to work in a fast-paced, dynamic environment
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong attention to detail and organizational skills
- Ability to lift, transfer, and reposition clients safely and effectively
