As a full‑time therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence from the comfort of their homes. Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals. Develop and implement a rehabilitation program consistent with the physician’s Plan of Treatment and the goals of the patient team. Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation. Document patient observations, interventions, and evaluations promptly and thoroughly. Report patient progress to the physician, Clinical Manager, care team, patient, and family; submit all required evaluations, plans, progress notes, and discharge summaries. Required Qualifications Degree from an accredited Physical Therapy Program approved by the APTA Current and unrestricted Physical Therapy license Current CPR certification Experience collaborating with a team of health‑care professionals Preferred Qualifications One year of experience as a Physical Therapist Pay Range $59.00 – $83.00 per visit/unit $93,000 – $128,000 per year (base pay) Additional Information TB Statement: This role is considered patient‑facing and is part of Humana’s Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB. Driving Statement: You must have a valid state driver’s license and maintain personal vehicle liability insurance in accordance with state minimum required limits.
