Travel RN – Home Health / Case Manager

Location: Barre, Vermont

Start Date: ASAP
Contract: 13 Weeks
Shift: Days | Monday–Friday (Weekend rotation approximately every 4th week)

Requirements

• Active Vermont RN License (required at time of submission)
• Minimum 1 year of Home Health RN experience
• OASIS experience required
• Previous Home Health travel experience preferred
• Case Management experience preferred
• Homecare Homebase (HCHB) experience preferred
• Current BLS Certification
• COVID-19 Vaccination Required (No Exemptions)
