Job Summary

This position offers a rewarding 13-week contract opportunity for a dedicated Licensed Vocational Nurse (LVN) at Modoc Medical Center's Skilled Nursing Facility (SNF) in Alturas, California. The LVN will play a crucial role in providing comprehensive, individualized nursing care to residents, working collaboratively with physicians, nursing staff, residents, families, and other facility departments.

The successful candidate will report to the Director of Nursing and be responsible for a range of clinical duties, including patient assessment, medication administration, treatment execution, and emergency response. This role emphasizes a patient-centered approach, ensuring high standards of care and effective communication within the healthcare team to optimize resident well-being and progress.

Key Responsibilities

• Conduct individualized patient assessments, prioritizing data collection based on immediate patient needs and facility protocols.
• Develop and implement patient-centered plans of care in collaboration with the interdisciplinary healthcare team.
• Administer medications and perform treatments as ordered by physicians, ensuring accuracy and timeliness.
• Execute assigned clinical tasks, including EKGs, blood draws, and IV starts.
• Initiate and perform emergency resuscitation measures according to established protocols and respond appropriately to emergency situations.
• Document patient assessment findings, responses to interventions, and progress towards problem resolution accurately and promptly.
• Provide individualized patient and family education regarding care plans and health management.
• Adhere strictly to facility policies and procedures for medication administration, narcotics control, safety, and infection control.
• Maintain a clean and organized work environment, including patient rooms and supply areas.
• Report patient conditions to appropriate personnel during each shift and ensure continuity of care through walking rounds.
• Assist with direct patient care, ensuring call lights are answered promptly and CNA-assigned tasks are completed and documented.
• Collaborate effectively with other departments to coordinate patient transfers, diagnostic tests, and specimen collection.
• Participate in the orientation of new personnel and contribute to staff education as needed.

Required Qualifications

• Active California Licensed Vocational Nurse (LVN) license.
• Current Basic Life Support (BLS) certification.
• Ability to communicate effectively in English, both verbally and in writing.
• Adequate sight and hearing to perform all job functions.
• Good physical health and stamina, with the ability to walk, bend, lift, and perform manual dexterity tasks required for patient care.

• Demonstrated interest in continuing education and self-development.

Skills & Competencies

• Clinical Skills: Strong patient assessment, critical thinking in emergency situations, EKG administration, phlebotomy (blood draws), IV insertion, medication administration, infection prevention, and proficient use of medical equipment.
