SMI Case Managers EMPACT

You can view all our job openings at: Sign-on bonus up to $2000 (limited time incentive) What you’ll do: We have multiple opportunities for SMI Case Managers at our Phoenix location. As a member of the clinical team, the SMI Case Manager will complete assessment and service plans. They will implement services to consumers identified as Severely Mentally Ill in accordance with their individualized service plan (ISP) while communicating and documenting their progress toward their recovery. Essential Responsibilities: Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with the consumer, their family, behavioral health, medical and dental providers, community resources, and others in ensuring that all services prescribed in the individualized service plan are implemented. Provide assistance in maintaining, monitoring, and modifying covered behavioral and medical health services. Provide brief telephone or face to face interactions with a person, family, or other involved party for the purpose of maintaining or enhancing a person’s functioning. Provide assistance in finding necessary resources other than covered services to meet basic needs. Serve as a point of contact to ensure ongoing collaboration including the communication of appropriate clinical information with other involved parties including medical integration as appropriate and coordination of care with a person’s family, behavioral and general medical and dental health care providers, community resources, and other involved supports including educational, social, judicial, community, and other State agencies. Ensure the provision of all covered services identified on the service plan; referrals to community resources as appropriate and coordination of care activities related to continuity of care between levels of care and across multiple providers, services and supports. Provide outreach and follow-up of services including, but not limited to, integrated care, crisis, and missed appointments to ensure adequate resources are available and in place. Provide transportation to the consumer as appropriate and determined by the clinical team. Perform all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluations. May sign off on service plans and annual updates completed for people on their own caseloads. Collaborate with the person and his/her family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option. Maintain the person’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning). Provide continuous evaluation of the effectiveness of treatment through the ongoing assessment of the person and input from the person and relevant others resulting in modification to the service plan as necessary. Adhere to the agency's policies and procedures. Comply with the agency's standard of care for service delivery. Assist with Quality Management, Quality Assurance, and Utilization Review. Perform additional related duties as assigned by Director. What you’ll provide: Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience. Excellent oral and written communication skills Detail Oriented and strong organization skills. Able to be flexible with workload and availability Strong internal and external customer service skills. Ability to exercise independent judgment and maintain appropriate professional boundaries. Ability to maintain strict confidentiality. Strong clinical and therapeutic skills and the ability to accurately document events, provide detailed records for members/clients and work with a multidisciplinary team. Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender, or sexual orientation. Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners. Strong computer skills including experience with Microsoft Office. A valid AZ Driver's License with no major infractions in past 39 months is required. Regular access to reliable transportation is required. Bi-lingual (Spanish) speaking ability a plus. Additional Requirements and Responsibilities: Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all regardless of age, race, creed, gender or sexual orientation. Professional conduct consistent with ethical professional practice and the policies and procedures of the agency. Professional demeanor and positive working relationships with internal/external team members, stakeholders, and community and business partners. What you’ll experience: The typical work environment will be working indoors or community-based work, working out in the community. This position requires the candidate to be 21 years old or older at time of employment. A fingerprint clearance card is a requirement of employment. Employees must be able to lift up to 15 pounds at times. While performing the duties of this job, the employee may be regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard. If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
