Schedule Information

Monday-Friday
Hours scheduled between 8:00 AM and approximately 4:30-5:00 PM
Up to 10-12 hours per week
Weekly hours may vary based on patient and operational needs

Occupational Therapist Requirements

Active Occupational Therapist license valid in Indiana
Degree from an accredited occupational therapy program
Education and professional license verification required
Previous home-health occupational therapy experience required
Previous OASIS documentation experience required
Strong patient-evaluation and treatment-planning skills
Accurate clinical documentation and recordkeeping abilities
Effective communication and interdisciplinary teamwork skills
Ability to work independently in patients' homes
Compassionate and patient-centered approach to care

Key Responsibilities

Complete occupational therapy evaluations in patients' homes.
Assess functional abilities, safety risks, and rehabilitation needs.
Develop and implement individualized occupational therapy plans of care.
Provide therapeutic activities to improve daily living skills, mobility, strength, coordination, and independence.
Educate patients and caregivers about treatment plans, adaptive techniques, and home-safety practices.
Complete OASIS assessments and other required home-health documentation.
Monitor patient progress and modify treatment plans as clinically appropriate.
Coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team.
Support safe discharge planning and continuity of care.
Maintain accurate and timely clinical records.
Follow all patient-safety, infection-control, and agency procedures.
Perform additional occupational therapy duties as assigned.
