Physical Therapist (PT) OR Physical Therapist Assistant (PTA) - Home Care | Per Diem / 1099 Contract

We are seeking a licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) to provide private, in-home physical therapy services in Southampton, NY and the surrounding area

This is a flexible, per diem / 1099 independent contractor opportunity ideal for a clinician looking to supplement an existing schedule while providing high-quality, individualized care in the home setting. Visits are offered on an as-needed basis, with flexibility based on clinician availability.

About the Role

Clinicians provide one-on-one physical therapy services in patients' homes. Visits are offered based on patient needs, geographic coverage, and clinician availability.

This is an as-needed position with no fixed weekly schedule or guaranteed number of visits . Clinicians may accept available visits based on their availability.

Responsibilities

Physical Therapists (PT):

• Perform physical therapy evaluations and reassessments
• Develop and update individualized plans of care
• Provide skilled physical therapy treatment in the home
• Address mobility, balance, gait, strength, safety, and functional independence
• Educate patients, families, and caregivers
• Complete timely and accurate clinical documentation
• Communicate with other members of the patient's care team as appropriate

Physical Therapist Assistants (PTA):

• Provide skilled physical therapy treatment consistent with the established plan of care and applicable supervision requirements
• Carry out therapeutic exercise, gait, balance, mobility, strengthening, and functional activities as directed by the PT
• Monitor and document patient response and progress
• Communicate patient changes or concerns to the supervising PT
• Educate patients and caregivers as appropriate
• Complete timely and accurate clinical documentation

Qualifications

• Current New York State license as either a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
• Graduate of an accredited PT or PTA program
• Home-care, geriatric, or private-duty experience preferred
• Strong communication and documentation skills
• Ability to work independently and professionally in patients' homes
• Reliable transportation and ability to travel to patients in Southampton and the surrounding area

Position Details

• Location: Southampton, NY and surrounding areas
• Employment: Per diem / 1099 independent contractor
• Schedule: Flexible / as needed
• In-home patient visits
• No guaranteed minimum number of visits or hours
• Potential for additional visits as patient needs grow

Compensation

$125–$200 per visit , depending on clinician type (PT or PTA), visit type, experience, and the specific needs of the assignment.
