Role Description This full-time, on-site Primary Care Physician role is based at an Oak Street Health center in Waukegan, IL. The physician will provide comprehensive primary care to an adult and older adult Medicare population, including preventive care, chronic disease management, and acute care visits. Day-to-day responsibilities include conducting patient assessments, developing and adjusting treatment plans, coordinating care with an integrated care team, and maintaining accurate clinical documentation. The role involves participating in quality initiatives, using value-based care principles to improve outcomes, and engaging patients through education and shared decision-making. The physician will also collaborate with other clinicians and care team members to ensure continuity of care and support practice-level performance goals. Qualifications Candidates should possess strong skills in Primary Care and Family Medicine to deliver comprehensive, continuous care across a broad range of conditions. Candidates should possess skills in Urgent Care and general Medicine to address acute issues, manage complex cases, and provide timely interventions. Candidates should possess experience and clinical competence as Primary Care Physicians, including managing panels of adult and older adult patients. Medical degree (MD or DO) from an accredited institution and completion of an accredited residency in Internal Medicine, Family Medicine, or related field. Current, unrestricted medical license in Illinois, board certification or eligibility in an appropriate specialty, and valid DEA registration. Demonstrated commitment to value-based care, preventive medicine, and working with Medicare or older adult populations. Strong communication, teamwork, and interpersonal skills, with the ability to collaborate effectively in an interdisciplinary care model.
