RN Case Manager: Discharge & Care Coordination

Advocate Health is seeking a Registered Nurse Case Manager to join our Lakeland/Burlington team on a PRN basis. You will assess patient needs, coordinate services, plan discharges, and support utilization management across the hospital stay.
This role emphasizes collaboration with patients, families, and the care team to ensure safe transitions and compliance with regulatory standards. The ideal candidate has a current RN license, a preferred Bachelor's in Nursing, and at least 2 years of
