Job Title

Location

Key Responsibilities
- Conduct comprehensive assessments to develop individualized rehabilitation plans tailored to each resident’s needs
- Deliver expert physical therapy treatments and interventions to promote mobility, strength, and independence
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care and outcomes
- Educate residents and their families about treatment goals and strategies to support recovery at home
- Supervise and mentor Physical Therapist Assistants to ensure high standards of care and professional growth
Required Qualifications
- Valid Physical Therapist license in Oregon
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program
- Current CPR certification
- At least six months of experience in a long-term care or skilled nursing facility (new graduates encouraged to apply)
Preferred Experience / Skills
- Experience working in SNF or LTC environments
- Strong communication and teamwork skills
- Passion for helping residents regain mobility and independence.
