Responsibilities
- Provide plan-of-care assistance under the supervision of a licensed physical therapist, focusing on mobility and functional restoration for LTAC patients.
- Lead or assist with therapeutic exercises, gait training, transfers, and activities to promote independence.
- Monitor patient responses to treatment, document progress, and communicate changes to the supervising PT and nursing team.
- Prepare and apply therapeutic modalities as directed, ensuring patient safety and proper equipment use.
- Maintain accurate, timely documentation and participate in daily interdisciplinary rounds as required.
- Support a patient-centered, safety-first care approach and assist with positioning, ROM, strength training, and activity tolerance.

Requirements
- Active Physical Therapy Assistant license in Connecticut or eligibility for licensure (per job notes workflow).
- Associate degree in Physical Therapy Assistant from an accredited program.
- Prior experience in rehabilitation or LTAC settings is preferred.
- Ability to work the scheduled shift (08:00–16:30), with flexibility to meet patient needs.
