Occupational Therapist - Home Health

Home Health Occupational Therapist | Adventist Health Help patients regain independence and improve their quality of life from the comfort of their homes. We’re seeking a compassionate and dedicated Home Health Occupational Therapist to provide personalized therapy services across our home health and hospice communities. What You’ll Do: Evaluate patients and develop individualized occupational therapy treatment plans Provide therapeutic interventions to improve physical, cognitive, and daily living skills Collaborate with interdisciplinary healthcare teams to support patient recovery goals Establish and coordinate care and discharge plans for continuity of treatment Complete accurate and timely clinical documentation Supervise therapy assistants and support staff as assigned What You’ll Bring: Current Occupational Therapy license in applicable state(s) of practice Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) required BLS/CPR certification required Valid driver’s license and reliable transportation required Strong communication, organization, and patient care skills Passion for helping patients achieve independence in their home environment Join a team committed to compassionate, patient-centered home care- where your expertise helps improve lives every day. #J-18808-Ljbffr
