Licensed Practical Nurse-Hospice

LPN For Hospice Team

SALMON Home Care is seeking a caring and compassionate LPN to join our growing Hospice Team.

The Hospice LPN is responsible for the following duties:

Conducts job responsibilities in accordance with the standards set forth in the SALMON Health and Retirement employee handbook, policies and procedures, applicable federal and state laws and applicable professional standards.

Must be able to work cooperatively as a member of a team.

Keeps confidential all patients information received.

Responsible for documentation of patient condition, records in the clinical record and reports all observed symptoms, reactions, treatments and changes in patient condition.

Administers and charts medications as ordered by physician.

Assists physician with examination, treatments, specials tests and procedures as defined within the scope of practice.

Obtains, carries out and follows physician orders

Assists with rehabilitations of patients in accordance with the medical plan

Directs patient care for assigned patients

Exercises independent judgement and decision making

Communicates effectively with patients and their families, physicians and staff.

Communicates in team conferences, development of nursing plans and team shift reports.

Assists in assessing, admitting, discharging and transferring of patients

Complies and performs with scope of practice as defined by state Nursing Practice Act.

Completes all documentation as required by local, state and federal guidelines

Qualifications:

• Nursing Degree form an accredited college and current LPN license in the State of Massachusetts.
• Minimum of 2 years' experience in a clinical setting
• Excellent organizational skills
• Ability to work under stress and in emergency situations

• Meets federal and state licensing requirements.
• Ability to communicate in English effectively through verbal and written means.
• Ability to work under stress and in emergency situations.
• Understands principles of growth and development for the entire lifespan with the ability to provide care appropriate for adult and geriatric patients as appropriate to the patient population.
