Clinical Case Manager

Marian House is seeking a Clinical Case Manager to support women and families in our Transitional Family Program. This role combines counseling, case management, and advocacy to help families achieve stability and long-term independence. What You’ll Do Provide weekly counseling and case management to families Develop and monitor Individual Recovery Plans (IRPs) Support residents with housing, healthcare, and recovery resources Coordinate care with internal staff and community partners Facilitate intake, goal‑setting, and progress reviews Maintain accurate, confidential documentation Participate in team meetings and on‑call rotation What We’re Looking For Bachelor’s or Master degree in Social Work, Counseling, Psychology, or related field required LMSW, LGPC (or equivalent MD licensure), CPRS Preferred 3–5 years of counseling/case management experience Experience with trauma‑informed care, substance use, and co‑occurring disorder Strong organizational, communication, and advocacy skills Why Marian House?
