2 days LPNS 6a-6p 12s 6 weeks, Variable - Days, 36HRS. LTC / Nursing Homes Responsibilities Provide licensed practical nursing care to residents in LTC settings. Assess resident conditions and report changes to the RN and care team promptly. Administer medications and document interventions accurately in charts. Develop and follow individualized care plans aligned with resident goals. Monitor and maintain infection control and safety standards on unit. Document all care activities timely and in compliance with regulations. Collaborate with multidisciplinary team to optimize resident outcomes. Engage in performance metrics tracking to meet quality and productivity targets. Maintain on-site presence and contribute to a high-visibility, results-driven environment. Educate residents and families about care plans and treatment expectations. Demonstrate willingness to learn and pursue professional development opportunities. Experience/Qualifications Active LPN license in good standing with state board. Experience in long-term care or LTC settings preferred. Administer medications and document interventions per policy and regulations. Assess resident conditions and report changes promptly to RN. Compliance with privacy, regulatory, and charting standards. Develop and follow individualized care plans aligned with goals. Strong infection control and safety standards adherence.
