Physical Therapist needed for a travel assignment in a skilled nursing facility setting in Milford, New Hampshire. Start date 08/31/2026 for a 13-week assignment with guaranteed 35 hours per week. Responsibilities
- Conduct initial evaluations and develop individualized physical therapy plans of care for residents.
- Implement evidence-based interventions to improve mobility, strength, balance, gait, and functional independence.
- Reassess residents regularly, document progress, and adjust the care plan as needed.
- Educate residents and families on home programs, safety, and recovery expectations; collaborate with other care team members to optimize outcomes.
- Maintain accurate patient records and documentation in the appropriate health information system.
- Adhere to safety, infection control, and privacy standards; participate in interdisciplinary care planning and discharge coordination.

Requirements
- Active physical therapy license (state licensure required to practice)
- Ability to work in a skilled nursing facility setting
- Strong communication and documentation skills
- Ability to collaborate effectively with an interdisciplinary team
