Key Responsibilities

• Provide skilled physical therapy evaluation and treatment services to patients in a skilled nursing facility setting
• Develop and implement individualized treatment plans in accordance with patient needs and clinical goals
• Document patient progress and maintain accurate clinical records; experience with Casamba EMR is highly preferred
• Must be open to floating between worksites as operationally required
• Collaborate with interdisciplinary care team to support optimal patient outcomes

Required Qualifications

• Active California Physical Therapist (PT) license required
• Minimum of 1 year of SNF (skilled nursing facility) experience required
• Standard credentialing requirements must be met prior to start
• Ability to work day shift hours and float as needed

Preferred Experience / Skills

• Experience with Casamba EMR documentation system strongly preferred
• Prior experience floating between multiple care sites
