Clinical Nurse Educator

Job Summary

The Cardiac Nurse Educator provides total comprehensive care and education to CV Fitness Phase II cardiac patients. This position works with patients, physicians, significant others, and ancillary departments to develop a comprehensive multi-disciplinary plan of care for the patient.

CARDIAC NURSE EDUCATOR, FLEX Proficiency Level

COLLABORATION: Collaborates with the patient, significant others, physicians, and ancillary departments to become a multi-disciplinary health care team whose goal is to provide care that is continuous an dwell coordinated. Prioritizes and organizes workassignments and adjust priorities based on changing situations. Maintains a therapeutic relationship with patients and family while providing care. Anticipates potential discharge problems and consults and refers to other Health Care professionals. Documents all patient care as required notifying the health care team plan of care, changes in condition, the patient progress, the discharge plan and other relevant information.

EDUCATION: Provides for actual and anticipated patient/significant other education needs and includes education to meet these needs in the plan of care. Integrates primary and secondary diagnosis into the education plan individualizing educational plan based on age, cultural and spiritual values. Assesses learning needs, readiness to learn and actual or potential barriers to learning. Incorporates education into daily patient activity (e.g. medications, equipment, food-drug interactions). Participates in in service and training of family or significant others as needed. Provides for actual and anticipated patient/significant other education needs and includes education to meet these needs in the plan of care.

PATIENT CARE: Complete initial assessment of Phase II outpatients according to departmental standards including assessment for potential risk factors. Recognizes and integrates patients' special needs and individualizes care acting as patient advocate and upholding patient rights. Assumes accountability for the delivery of patient care and adheres to infection control standards specific to patient care. Provides total comprehensive care to patients by applying the nursing process in an organized and systematic manner according to institutional policy.

Education

* 2 year / Associate's Degree (Required)

4 year / Bachelor's Degree (Preferred)

Master's Degree (Preferred)

Certification / Licensure / Registration

*State Registered Nurse License (Required)

ACLS Certification (Preferred)

Experience and Skills

*2 - 4 years Ability to use office equipment such as fax machine, copier, computer, etc. (Required)

*2 - 4 years Critical care or telmetry experience. (Required)
