PSH: Scattered Site Housing Case Manager

PSH: Scattered Site Housing Case Manager 40 hours per week/full-time, non-exempt Case Manager. Provides case management and housing navigation assistance to help the homeless return to living in traditional and permanent housing. Responsibilities: Responsibilities and tasks include a wide range of case management activities working with the homeless population – primarily with adults. Housing navigation case management tasks helping the homeless secure housing and look for available housing/rental units, coordinating housing with landlords, connecting the homeless with employment, community resources and government and health benefits, and developing case plans with clients in order to help them stabilize and resume healthy functioning; coordinating with and participating in coordinated entry rapid rehousing activities; resume preparation, job coaching and connection to employment opportunities; food distribution and connection to resources during street outreach activities; collection of data and shared responsibility for tracking outcomes and program performance measures; community advocacy and participation in collaborative task force activities; presenting and providing information to community members and organizations about the program; working with the media; and coordinating with inter-agency programs and activities. Bachelor’s Degree or higher in Human Services or related field preferred; and a minimum 1 year case management experience or related professional licensure or certification i.e.; LCSW, MFT, CADAC, LVN, LPT, etc.; Knowledge of HMIS and Housing First model preferred but not required; Experience with homeless, vulnerable and/or diverse population and the knowledge surrounding poverty and social factors involved; Ability to understand the needs of formerly homeless people with disabilities and to develop collaborative goals towards greater self-sufficiency and independence in the greater community; Respond to all persons in a calm, supportive and professional manner; Knowledgeable of the dynamics of chemical dependency, mental disorders and the effects of homelessness; Excellent verbal and written skills; and Working knowledge of Microsoft Suite, computer literacy and willingness to learn other applications. Must have own working transportation with liability insurance to conduct job-related travel. Must satisfactorily pass the company’s background, DMV and reference check. Must meet insurance carrier’s requirements for approval to drive. Bi-lingual a plus but not required. Schedule & Compensation: Work hours are Monday – Friday, 9-5 although there will be some nighttime and weekend hours on occasion to attend community events and/or to manage any off-hour client crises that may arise. Starting salary range is $25.00 – $28.00 per hour (DOE) with medical and dental benefits provided, 403b participation and match, as well as generous paid time off. Position begins asap. This is not a remote position.
