Required Qualifications
Registered Nurse, Home Health Experience: 2 years Licenses: RN(Compact Accepted)(Copy needed) Certifications: BLS-AHA(Copy needed) SSN Required DOB Required References: 2 References in entire work history EPIC and OASIS experience
Case Manage 20-25 patient with an expectation of 5-6 visits per day
