We're currently hiring a Full-Time Physical Therapist to join our dedicated Home Health team at Frederick Health Home Care in Frederick, MD.

Qualifications

• Degree from a CAPTE-accredited Physical Therapy program
• Licensed (or eligible) in Maryland
• BLS certification (American Heart Association)
• 1+ year of Home Health experience preferred
  (Inpatient or skilled nursing experience also considered)
• Proficiency with EMR systems
• Strong communication, time management, and flexibility

Key Responsibilities

• Deliver high-quality, outcomes-driven physical therapy services
• Accurately document care using a web-based EMR system
• Educate patients and families on treatment plans
• Meet clinical, departmental, and regulatory standards
• Foster a positive, team-oriented work environment

$64.75 - $135 Per Visit
