Job Description

An opportunity is available for a skilled and compassionate Registered Nurse with recent Home Health experience. In this field-based role, youll deliver high-quality nursing care directly in patients homes, supporting individualized care plans while collaborating with a broader interdisciplinary team. Schedule:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Daily travel to patient residences (mileage reimbursement provided)
• Caseload: 25-32 points per week (equivalent to 35 patient visits per day)

Key Responsibilities:

• Complete comprehensive OASIS assessments and tailor care plans to each patients needs
• Deliver direct skilled nursing care, such as wound care, IV therapy, and medication administration
• Monitor patient progress, respond to changes in condition, and communicate updates to providers
• Provide education and resources to patients and families regarding treatment and disease management
• Accurately document all patient interactions and interventions using Home Care Home Base (HCHB)
• Collaborate with therapists, social workers, and home health aides for seamless, coordinated care
• Participate in case conferences and contribute to ongoing improvement of care standards

Required Qualifications:

• Active RN license (state of practice)
• Minimum of 1 year recent Home Health field experience
• Proficient with Home Care Home Base (HCHB) and OASIS documentation
• Valid drivers license, reliable transportation, and auto insurance
• Ability to work autonomously, manage caseload, and demonstrate strong clinical skills

Preferred Qualifications:

• BSN
• Previous experience with Medicare guidelines and managing a 25-32 point caseload

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
