Physician (MD/DO) for In-home Visits

Position Overview Signify Health is seeking physician (MD/DO) contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. The role offers flexible scheduling and competitive compensation. Responsibilities Conduct in‑home visits focusing on overall health, well‑being, and potential barriers to care. Complete a medication and medical history review, as well as a physical evaluation, using a pre‑populated, patient‑specific form. Provide findings to the member’s health plan and care team without prescribing medication, ordering lab tests, or altering current treatment regimens. Assist in scheduling other appointments or referrals as needed. Maintain accurate documentation for each assessment. Compensation & Schedule Earn up to $3,000–$5,000 weekly (average 6–9 visits daily, $100 base rate per completed assessment). Increase per‑visit rate up to 135% using the Market Multiplier with a Medicaid license or willingness to enroll. Earn additional payment via pay‑per‑test model for qualifying members. Shifts available Monday–Sunday (no nights or weekends required); short‑term or long‑term basis. Availability incentives based on weekly availability. Airfare, hotel, and rental car available depending on eligibility. Incentive rates subject to change. Area exclusions may apply. Qualifications State medical license required. MD or DO degree.
