We are dedicated to making a difference in the lives of employees, offering the opportunity to be associated with caring teammates, and creating positive contributions in each community we serve.What you must have:Current state registered nurse licenseCurrent state driver's license, reliable transportation, and provide ongoing valid and current auto liability insuranceTwo years of experience as an RN preferred in a healthcare/clinical settingFull time employee benefits:Competitive payMedical, dental & vision insurancePaid time offPaid holidays401k with up to 4% employer matchingTuition reimbursementCompany car for qualifying individualsMileage reimbursementWhat you will do:Provide telephonic triage to hospice patients and their families during after-hours, weekends, or at other times when office is closed or at the request of the executive directorEnsure after-hour visits are scheduled with the on-call team in a timely mannerEnsure documentation is completed and submitted to the site at the end of each shift to ensure continuity of care for the weekday teamManage referrals that come in over the weekend and after-hoursProvide education and support to patients and families/caregiversConduct patient visits as needed (admissions, routine, pronouncements, tuck-ins)Promote positive, respectful communication to all staff, referral sources, patients and familiesPerformance of other duties as requiredEnsure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and proceduresIt is this agency's policy to provide equal employment opportunities without regard to age, race, color, religion, military status, gender preference, sex, marital status, national origin or disability.
