Responsibilities Lead and manage a team of social workers , overseeing daily operations and ensuring that residents receive timely and effective support. Develop, implement, and maintain social services programs , including assessment, care planning, and discharge planning. Collaborate with interdisciplinary teams , including nursing, therapy, and administration to ensure comprehensive care and services. Maintain accurate records and reports , including social work notes, care plans, and other required documentation. Participate in organizational quality improvement initiatives , contributing to the development and implementation of quality care initiatives. Stay up-to-date with industry trends and regulations , ensuring compliance with relevant laws and standards. Requirements Minimum 2+ years of experience in social work, preferably in a healthcare setting. Knowledge of social work principles, theories, and practices. Effective communication, leadership, and problem‑solving skills. Ability to work in a fast‑paced environment with multiple priorities and deadlines.
